Sponsor Certificate

Congratulations on being asked to sponsor someone along their faith journey! What an honor to help establish, grow, and be a witness to someone’s relationship with God. We are here to help you in all ways to uphold your commitment with this privilege.

Please answer the following questions to help us adhere to the guidelines set forth by the Catholic Church for obtaining a Sponsor Certificate.

If you have any questions, concerns, or uncertainty, please reach out to us so we can guide you through the process. We will take good care of you and your needs. Once your form is submitted, please allow 3-5 business days for us to process your request. The Sponsor Certificate will need to be signed by the sponsor before it is sent to the requesting church. Please note that Sponsor Certificates are valid for 6 months.

I am a Catholic in good standing and I am a registered and participating member of St. Ambrose.(Required)
I have received the three Sacraments of Initiation: Baptism, Holy Communion, and Confirmation.(Required)
I regularly participate in Mass on Sundays and Holy Days of Obligation and give witness to my faith in Jesus Christ by receiving him in Holy Communion.(Required)
I am living my Christian vocation as a single person, a validly married person, a religious, or clergymen.(Required)
I actively strive to live out my commitment to Christ and to the community life of the Church by my loving response to those with whom I come in contact daily.(Required)
