I hereby grant permission for the above named participant to attend the 1:38 team retreat at Saint Ambrose Parish in Brunswick, OH (and off-site locations), on August 2-3, 2026 and I consent to my child’s participation in said 1:38 retreat. I understand I am responsible for transporting my child to and from the event at Saint Ambrose Parish in Brunswick, OH. The event will be supervised by Saint Ambrose Parish staff and VIRTUS-trained adult chaperones. I understand that my child will be transported to and from off-site locations by a Brunswick School District school bus and/or parent driver (personal vehicle).
In consideration of my child being allowed to participate in the 1:38 team retreat, on behalf of my child and myself, I hereby assume all risks in connection with the service retreat and I further release, discharge, and/or otherwise indemnify the Diocese of Cleveland, the Bishop of the Roman Catholic Diocese of Cleveland, Saint Ambrose Parish, employees and volunteers from all claims, judgments, liability by or on behalf of my child, myself and my spouse for any injury or damage due to the child’s participation in the event including all risks connected therewith whether foreseen or unforeseen.
Furthermore, I acknowledge that it is my responsibility to provide adequate health insurance for my child. I fully understand what is involved in the event and I understand that I have the opportunity to call Caitlin Arbogast or Jillian Langley (330.460.7387) with any questions or concerns.
